New Patient Health History Form - Required

This lets us know the history and current state of your health. Please feel free to download this form and fill it out before your first visit. Once you have completed the form you can print it out and bring it with you, fax it to us before your visit or email it to drsg@drsgoodwin.com. Make sure to let us know if you have any questions or concerns.

HIPPA Compliance Form - Required

This form is required by the State of California concerning patient rights. Please feel free to download this form and read it before coming in for your visit. Once you have read the form you can either fill it out and print it out or you can fill the form out, save it and email it to drsg@drsgoodwin.com. Let us know if you have any questions or concerns.

Member Wellness Registration Form - Optional

This form can be filled out to register for access to the member wellness section of our website. You can also sign up for our monthly newsletter to keep up on current health issues and news and events in our office. You can print it out and bring it in to our office or Click Here to register online! The online newsletter sign-up is also on the right.
